Some nice tips. Especially agree with improving CTR - a few tweaks to ad group relevancy may increase average CTR from say 1% to 1.5%, which would deliver a massive 50% more visitors, a very hard feat to achieve from Yahoo or Bing.
One other suggestion I might add to your list would be international targeting. Once a client's PPC return starts to plateau in their domestic market, I usually look at opportunities overseas, which can often be extremely profitable, especially if no PPC is currently being conducted internationally.
